StimLabs Privacy Policy

INTRODUCTION

Stimlabs® LLC (“StimLabs”) respects your privacy and is committed to protecting your personal information. This Privacy Policy (“Policy”) describes our policies and practices regarding collection, use, and disclosure of personal information we receive when you:

  • visit our website, stimlabs.com (the “Website”),
  • use our products or services, or
  • otherwise interact with us directly or interact with our business partners.

The above may be collectively referred to in this Policy as our “Services.”

CHANGES TO THIS PRIVACY POLICY

By using our Services, you consent to the data practices described in this Policy. We may update this Policy from time to time to reflect changes to our information practices. When we make changes to this Policy, we will post the updated version on our Website and indicate the date the Policy was last updated. We encourage you to review this page periodically to check for any updates. Your continued use of our Website or Services indicates your consent to the terms of our Privacy Policy at the time of use.

INFORMATION COLLECTION

We may collect the following types of information in the following contexts.

  • Information You Provide to Us: We collect information you provide directly to us. For example, we collect information when you fill out a form, send us an email, sign up to receive email updates, create an account, make a purchase, communicate with us via third party social media sites, request information, or otherwise communicate with us. The types of information we may collect include your name, email address, postal address, phone number, phone number, and other contact or identifying information you choose to provide. You may also provide payment card data to the extent you pay invoices through the Website, but note that this information is received directly by a third party payment provider. Website visitors may also submit additional information in the message field of our Contact Us webform, but we ask that you not provide sensitive information, including Protected Health Information (“PHI”).
  • Website Information Automatically Collected: In addition, we also use cookies and other tracking technologies to automatically collect information regarding your visit to our Website; please see the section below for additional information regarding this type of collection.
  • Patients: When healthcare providers (“HCPs) submit to us Insurance Verification Requests (“IVRs”), we receive personal information such as a patient’s first name, last name, date of birth, phone number, email address, health insurance information, and information relating to relevant health diagnoses and medical procedures. We receive Protected Health Information (“PHI”) pursuant to a business associate agreement under the Health Insurance Portability and Accountability Act (“HIPAA”). Additionally, we work with HCPs on case reports and testimonials regarding HCPs’ and patient’s experience with our products. In doing so, HCPs collect consent for PHI to be provided to us, if the information is not de-identified in advance.
  • Donors: Stimlabs contracts with hospitals and HCPs to partake in the collection of donated placental tissue. In doing so, donors sign a consent form that allows HCPs to provide Stimlabs PHI.
  • Job applicants: If you apply for a job with Stimlabs, you may provide basic personal information such as your name, email address, address, phone number, as well as additional information found on your resume or otherwise requested in our application such as your employment and education history, and work authorization status.
  • Business contacts: If you interact with Stimlabs in a business-to-business context or in the context of investor relations, we may collect your business contact information such as your name, email, and phone number.

COOKIES & TRACKING TECHNOLOGIES

A cookie is a small text file that is stored on your browser or the hard drive of your computer. We use cookies and similar technologies such as tracking pixels and web beacons to collect user activity information including your IP address (often considered personal information), location, browser and device characteristics, operating system, language preferences, referring URLs, information about how and when you visit the Website, and other technical details. We use the following cookies:

  • Necessary cookies. These are cookies that are required for the operation of the Website.

Most web browsers automatically accept cookies, but you can modify your browser settings to decline cookies if you prefer. If you use your browser settings to block all cookies, you may not be able to access all or parts of our Website.

Some internet browsers have “Do Not Track” or “DNT” features which, when turned on, send a signal to a website that the individual visiting the website does not wish to be tracked. Such browser features and industry standards are not uniform, so our Website does not respond to DNT signals, but you can set your browser settings to restrict the placement of cookies as described above.
